針灸改善中風後遺症：神經機制與核心穴位之敘述性綜論 ACADEMIC EXCHANGE · STROKE & ACUPUNCTURE 摘要與臨床–機制性討論基於：Zhang et al., Brain Research Bulletin, 2024。 說明：本文章為專業學術交流之綜合整理，用於討論針灸在中風後神經功能障礙之研究證據， 並非個別醫療建議或治療處方。 1. 前言（Introduction） 中風為導致長期失能與殘疾的主要原因之一，常伴隨多面向的神經後遺症，包括運動功能障礙、 吞嚥困難、感覺異常、以及精神或認知方面的受損。這些狀況與缺血性神經損傷、結構與功能性腦網路中斷、 過量谷氨酸（glutamate, Glu）所引發的興奮毒性、神經發炎，以及神經可塑性受損等機轉密切相關。 過去二十年間，針灸逐漸受到關注，作為中風後復健的輔助策略之一。基礎與臨床研究顯示， 針灸能調節神經可塑性、重組皮質網路、調控 Glu 與 γ-胺基丁酸（GABA）等神經傳導物質、 減輕發炎級聯反應，並改善局部腦血流與能量代謝。本文綜合相關證據，聚焦針灸在中風後 運動、吞嚥、感覺、情緒、認知及語言功能障礙中的神經機制與主要用穴。 2. 運動功能障礙（Motor Dysfunction） 2.1 中風後偏癱（Post-stroke hemiparesis） 中風後偏癱是最常見且致殘性極高的後遺症之一，特徵為病灶對側肢體自主運動能力下降， 臨床表現包括肌肉鬆弛、肢體活動遲緩，以及患肢日常生活活動困難。功能恢復高度仰賴 上下肢運動神經迴路的重建，而針灸與電針在此領域已被廣泛應用。 在臨床實務中，ST36（足三里）與GB34（陽陵泉）是中風後肢體運動 復健中常用的核心穴位，單用或合併其他穴位均見報告。頭皮針方面，雙側 「頂顳前斜線」常用於中樞性肢體運動障礙。功能性磁振造影研究顯示， 刺激 GB34 可以調節多個與上下肢、面部、舌頭與喉部運動及感覺意象相關之腦區的功能連結， 且在中風早期（約一個月內）反應更為顯著。 2.1.1 臨床證據 多項隨機對照試驗（RCT）指出，於傳統復健之外加上頭皮針治療的患者，其 Fugl–Meyer 運動評分（FMA）、徒手肌力評估（MMT）、日常生活活動（ADL）、Barthel 指數與 NIH 中風量表 （NIHSS）等指標改善幅度，均優於單純復健組。研究亦發現，基線損傷較輕者在針灸治療後， 對側上下肢運動皮質區域出現更廣泛的活化，凸顯殘存神經可塑性對治療反應的重要性。 2.1.2
